REGISTRATION FORMS:
Please fill out the following forms:
New Student Registration Form 2023-24 (All Students)
Transportation Form (All Students) Due by July 27 to be included in community stop query.
Volunteer Form (All Students)
Student & Family Compact (All Students)
Study Center Agreement (Optional - please sign if you intend to use Study Center)
Print these Documents:
Please complete these forms with appropriate signatures, scan, save with student last name and first initial (i.e. RasmussenN) and email to info@cybervillageacademy.org or mail to CVA 3810 E 56th St. Minneapolis, MN 55417.
Forms included are: Emergency/Medical, Immunizations(All New Students), Race/Ethnicity(MDE required), Application for Educational Benefits, Home Language Questionnaire, Residency Form
- MDE Immunization Records & Parent Letter (All New Students)
- Race & Ethnicity Document (All Students - MDE)
- Residency Form (All Students)
- Home Language Questionnaire (All Students - MDE)
- Educational Benefits Form (All Students - School funding and for determining eligibility for free or reduced cost lunches, etc.)
- Medication Authorization & Emergency Contact Form (Optional - if needed)
- Student Medication PRESCRIBER Authorization (Optional - if needed)
