Please download these forms. Forms are fillable PDF's or need to be printed for appropriate signatures. Forms can be completed, saved and/or scanned with student last name and first initial (i.e. RasmussenN) and emailed to jhernandez@cybervillageacademy.org or mailed to CVA 3810 E 56th St. Minneapolis, MN 55417, Attn. Jennifer.
- Educational Benefits Form (All Students - School funding and for determining eligibility for free or reduced cost lunches, P-EBT, etc.) (Required)
- MDE Immunization Records (All New Students): Physician office completes or printed immunization record (Required)
- Medication Authorization Form (Optional - if needed)
